Procedure Log App
The personal information that you enter into the Procedure Log app (your name, institution name, your email address, your date of birth, social security number, and your username and password) is stored in a secure manner within your iOS device but outside of the app (in the Keychain) and is not sent to me or to any other third party, but it can be backed up to your iCloud account in an encrypted format if iCloud Keychain is set to on. Entering your date of birth and social security number is optional—that information is used if one forgets his or her username or password for the app; one can enter their date of birth, social security number (or any other personal identifier) and the answer to a third question that one creates in order to see their username and password (accessed by tapping on "Forgot Username or Password" from the login view). If you delete the Procedure Log app from your iOS device, that information is still stored in the Keychain. One can delete that information by reinstalling the app, then tapping "Delete" when prompted to delete that information, then delete the app again.
The patient information that you enter into the Procedure Log app is secured by encrypting the data into a password protected ZIP file that is encrypted using the AES-256 encryption algorithm. This will occur whenever you log out of the app or when the app is no longer the active app. Any photos and signature attestations are also encrypted in a password protected ZIP file that is encrypted with AES-256.
The reports that can be produced are a PDF report that is password protected and encrypted using AES-128 and a CSV (comma-separated values) file that is encrypted by creating a ZIP file that is password protected and encrypted using AES-256 in order to protect your patient's privacy and meet HIPAA requirements, particularly when transferring the data from your phone to your computer or sending by email.
